Application Materials Checklist

- A completed Salem State College application form.
- A $35.00 non-refundable application fee made payable to Salem State College by check or money order.
- A completed Prism Information Sheet authorizing the processing of your application. There is no processing fee.
- A complete, official transcript documenting the award of a bachelor's degree.
- Official transcripts of any graduate courses taken or degrees held. If you are seeking transfer credit, you must complete a Transfer of Credit Form for each course requested. Salem State College will consider nine graduate credits for transfer into this program, as long as license is not also sought.
- Three completed Reference for Graduate Admissions forms; referees may attach a letter, but must also sign the form. Applicant must fill out and sign the bottom of the sheet.
- Official copy of scores from either the Graduate Record Examination (GRE) or the Miller Analogies Test (MAT) taken within the last five years, unless you already hold a master's degree. This test score must be sent directly to Salem State College at the time it is taken. (The testing center will direct you.) However, a receipt for having taken the test and a copy of the results, when the applicant receives same, must then be filed with the Jenmarc office in the student's file.
- Students for whom English is not a first language must submit the official results from the Test of English as a Foreign Language (TOEFL) in addition to the GRE or MAT test results.
- A photocopy of your current Teacher's License and copy of your scores on the Massachusetts Tests for Educator Licensure, if you have taken them.
- A 500 to 1000 word, type-written, double-spaced statement of purpose addressing the following questions:
- What factors in your personal and professional history have influenced you in the past and now lead you to seek admission to this program?
- What are your long term professional goals and how will this program help you to meet them?
- Is there any other information that you think would help the admissions committee to evaluate your application?
