Application Materials Checklist
Master of Education in Reading Degree Program

  1. A completed Salem State College application form.
  2. A $35.00 non-refundable application fee made payable to Salem State College by check or money order.
  3. A completed Prism Information Sheet authorizing the processing of your application. There is no processing fee.
  4. A complete, official transcript documenting the award of a bachelor's degree.
  5. Official transcripts of any graduate courses taken or degrees held. If you are seeking transfer credit, you must complete a Transfer of Credit Form for each course requested. Salem State College will consider nine graduate credits for transfer into this program, as long as license is not also sought.
  6. Three completed Reference for Graduate Admissions forms; referees may attach a letter, but must also sign the form. Applicant must fill out and sign the bottom of the sheet.
  7. Official copy of scores from either the Graduate Record Examination (GRE) or the Miller Analogies Test (MAT) taken within the last five years, unless you already hold a master's degree. This test score must be sent directly to Salem State College at the time it is taken. (The testing center will direct you.) However, a receipt for having taken the test and a copy of the results, when the applicant receives same, must then be filed with the Jenmarc office in the student's file.
  8. Students for whom English is not a first language must submit the official results from the Test of English as a Foreign Language (TOEFL) in addition to the GRE or MAT test results.
  9. A photocopy of your current Teacher's License and copy of your scores on the Massachusetts Tests for Educator Licensure, if you have taken them.
  10. A 500 to 1000 word, type-written, double-spaced statement of purpose addressing the following questions:
      1. What factors in your personal and professional history have influenced you in the past and now lead you to seek admission to this program?
      2. What are your long term professional goals and how will this program help you to meet them?
      3. Is there any other information that you think would help the admissions committee to evaluate your application?
